Unwanted noises perceived by the patient with No source in the external environment. Can be very distressing to the patient and is variously described as ‘ringing’ or ‘roaring’ in nature and the character the noise does not determine the site of disturbance. Causes include prebycusis, Menier’s syndrome, depression, vascular disease, drugs (salicylates, quinidine, aminophylline, indomethacin, caffeine) and acoustic neuroma. All cases are to be evaluated by specialist for a cause. In many cases, the cause cannot be determined.
Treatment
Where No causes is found,
a) Educate and reassure patient b) Relaxation techniques c) Background noise like music playing during night d) Tinnitus maskers e) Drugs – Betahistine 8-16 mg daily// Carbamezapine// Antidpressants // Sodium valproate. In severe acute tinnitus, IV Lignocaineis given as detailed under Migraine, and often proves very effective.
