Definition ‘A hallucination of motion’ when there is no  motion, or an exaggerated sense of motion in response to bodily movement, and is a cardinal symptom of vestibular disease.
   
General Measures Bed rest is necessary during the acute phase. Reassure the patient of possible adequate control of the distress with drugs.
To take only liquids until the vertigo is brought under control.
Labyrinthine exercises to improve compensatory mechanisms. I.e. basically engage in movements that precipitate vertigo, up to the point of nausea or fatigue. Under expert physiotherapist’s advise substantial improvement may be achieved in patients with chronic disabling vertigo.
Where some disability persists despite all therapy, get a walking stick or a walker.
The use of diuretics combined with low-sodium diet in cases of Meneier’s syndrome has not been proven effective and this therapy has declined. Tranquillisers, vasodilators, lithium and papavarine have all been tried and not proven any better than placebo.
   
Advice to Patient In acute vertigo, or chronic and recurrent and positional vertigo not to drive any vehicle until control of vertigo is achieved
Physical activity improvees the central nervous system’s ability to compensate for the vestibular dysfunction. Hence, once nausea and vomiting subside, keep physically active.
   
Follow Up Depends on the cause of the vertigo.
   
Inadequate Response Most cases of vertigo tend to last some days to weeks and need watching despite symptomatic improvement. It will be best to get an Otologist’s opinion at an early stage.
   
Prevention Avoidance of ototoxic drugs like aminiglycosides, aspirin, quinine, frusemide etc., in general, and particularly in those with history of aural problems.
