Indication & Dosage
 
 
Oral
SHORT-TERM MANAGEMENT OF INSOMNIA
Adult: 7.5-15 mg given at night.
 
Oral
PREMEDICATION IN SURGICAL PROCEDURES
Child: ≥6 mth: Single dose of 250-500 mcg/kg, up to a max of 20 mg. Younger patients (<6 yr) may require higher doses/kg than older patients. In obese paediatric patients, calculate dose based on ideal wt.

PREOPERATIVE SEDATION/ANXIOLYSIS/AMNESIA PRIOR TO OR DURING DIAGNOSTIC, THERAPEUTIC OR ENDOSCOPIC PROCEDURES
Adult: Initially, 1-2.5 mg given over 2 min, increased by increments of 0.5-1 mg at 2-min intervals as needed, until the desired endpoint is achieved.
 
Intravenous
PREOPERATIVE SEDATION/ANXIOLYSIS/AMNESIA PRIOR TO OR DURING DIAGNOSTIC, THERAPEUTIC OR ENDOSCOPIC PROCEDURES
Child: 6 mth-5 yr: Initial dose of 0.05-0.1 mg/kg given over 2 min, increased by small increments at 2 min intervals. Max total dose 6 mg. 6-12 yr: 0.025-0.05 mg/kg given over 2 min and increased by small increments at 2 min intervals. Max total dose 10 mg. In obese paediatric patients, calculate dose based on ideal wt. Elderly: Initially 1-1.5 mg given over 2 min, increased by increments of 0.5-1 mg at 2 min intervals as needed.
 
Intravenous
FOR INDUCTION OF ANASTHESIA
Adult: 150-200 mcg/kg by slow inj in premedicated patients and at least 300 mcg/kg for those who have not received a premedicant. Max 600 mcg/kg.
 
Oral
FOR INDUCTION OF ANASTHESIA
Child: >7 yr: 150 mcg/kg. In obese paediatric patients, calculate dose based on ideal wt. Elderly: 100-200 mcg/kg.
 
Intravenous
INDUCTION OF CONTINUOUS SEDATION FOR PATIENTS IN INTENSIVE CARE
Adult: Initially, 30-300 mcg/kg given as infusion over 5 min or by slow IV inj in steps of 1-2.5 mg every 2 min. US guidelines recommend an initial dose of 10-50 mcg/kg repeated at 10-15 min intervals until sedation achieved. Maintenance dose 20-200 mcg/kg/hr as IV inj or infusion.
 
Intramuscular
PREMEDICATION IN SURGICAL PROCEDURES
Adult: 70-80 mcg/kg (usual dose of 5 mg) given 20-60 min before surgery by deep IM inj.

Administration May be taken with or without food.
   
Precautions Paediatric patients; elderly; debilitated patients; chronic renal failure; may impair ability to drive or operate machinery.
   
Potentially Life-threatening 
Adverse Drug Reactions
Physical and psychological dependence with withdrawal symptoms; decreased tidal vol and resp rate; apnoea; headache, hiccough, nausea, vomiting, cough, oversedation; seizure-like activity (paediatrics); paradoxical reactions, kernicterus, hiccough; nystagmus.
   
Adverse Drug Reactions Respiratory depression, resp arrest; hypotension.
   
Interactions

Alcohol, opioids, barbiturates, other sedatives and anaesthetics potentiate the CNS depressant action.
